Home and long-term care

Kaiser Permanente provides help and support for those who require long-term or home health care. These services include skilled nursing, physical therapy and social work care at home, coordination of nursing home care, and hospice services.

Coverage for these types of services vary depending on the member's medical coverage agreement. Home Health Services is Medicare-certified.

Home Health Services

If you or someone you care for is temporarily unable to leave home because of health problems, Kaiser Permanente's Home Health Services can bring skilled nursing care, physical therapy, and occupational therapy to your home to help you work toward returning to independence.

The Home Health Services team includes:

  • Registered nurses
  • Physical therapists
  • Occupational therapists
  • Speech pathologists
  • Medical social workers, and
  • Home health aides

Hospice Program

Kaiser Permanente's Hospice Program offers physical, emotional, and spiritual care for patients who are in advanced stages of incurable illness.

Nursing homes

The decision to admit to a nursing home is made by your doctor. Admission can occur from the hospital, emergency department, or clinic. Covered skilled nursing care is usually provided in Kaiser Permanente-contracted facilities.
