Important member information

Member ID card

When you become a Kaiser Permanente member, you and each member of your family covered under your plan will receive a member ID card.

Use your ID number to:

  • Make or cancel appointments
  • Check in for an appointment
  • Order or pick up prescriptions
  • Get lab or X-ray services
  • Receive emergency care
  • Sign on to the member website

Benefits booklet (Certificate of Coverage)

Your benefits booklet details which benefits and services your health plan covers, the copays and coinsurance you'll pay for office visits and prescriptions, plus services your plan does not cover.

Note: This document is called the "Evidence of Coverage" for Medicare plans. You can read or download it online after you register on the secure member website.

To get a printed version, you can:

  • Fill out and return the request card included with your enrollment materials
