The Standard Option Plan

A low-deductible, low-premium medical plan that covers all your preventive care visits 100%. And there’s just a copay and no deductible for primary and specialty care visits. This plan also includes preventive dental coverage.

The High-deductible Health Plan (HDHP)

With a High-deductible Health Plan preventive care is covered 100% and preventive dental coverage is included. It’s our highest-deductible and lowest-premium catastrophic coverage plan.

As a HDHP member, you’re eligible for either a Health Savings Account (HSA) or a Health Reimbursement Arrangement (HRA) account. See HSA & HRA Facts for details. With these accounts, funds are deposited into your account to be used for qualified medical expenses.

In order for us to establish the correct account for you, you must complete the Health Savings Account (HSA) Eligibility Worksheet (PDF). Mail the completed form to:
