- How to quit smoking
- Controlling cholesterol
- Reducing stress
- Starting an exercise program
- Lowering blood pressure
You can also request record-keeping cards, reference lists, and patient education DVDs about selected diseases and conditions.
Classes and support groups
We can direct you to classes and support groups at Kaiser Permanente and in the community, and help you register. Learn about community agencies such as the Arthritis Foundation and the American Diabetes Association.
We can also help refer seniors to Medicare Advantage fitness benefits like SilverSneakers or EnhanceFitness.
Support for seniors and caregivers
We can provide information on home care, transportation, adult day care, respite care, and related topics.
Living wills and advance directives
Plan ahead for health care in case you can't make your own decisions because of an injury or illness. Get an advance directives booklet, forms for durable power of attorney for health care (DPOA), Living Wills, and Physician Orders for Life Sustaining Treatment (POLST).
