Changing your status

Changes in your household or employement? You may need to update your medical plan.

Personal or family changes

Individual & Family and employer-sponsored plans direct from Kaiser Permanente:

Contact Member Services if you change:

  • Your name
  • Address or phone number (Individual & Family plans only)
  • Dependents by birth, marriage, or adoption
  • Dependents who are no longer eligible

Individual & Family plans through Washington Healthplanfinder:

Contact Washington Healthplanfinder if you change:

  • Dependents by birth, marriage, or adoption
  • Dependents who are no longer eligible
  • Your income

Medicare:
Contact the Social Security Administration if you change your name, address, or phone number.

Employment status changes

You can keep your coverage even if you leave your job or lose eligibility under your medical plan.

Continue coverage:

  • The Group Conversion Plan leaves no lapse in coverage between your previous plan and new plan. Level of coverage and monthly premiums may vary. You must apply and make payment within 31 days of termination of your previous plan.
  • The Individual & Family Plan is a second option for continuing coverage.

Temporary coverage:
When you leave your job you may be eligible for COBRA, a continuation of your medical coverage and benefits plan.
